Main Duties and Responsibilities:
- Empower participants to understand, implement and review their NDIS Plans
- Facilitate NDIS participants, nominees and/or persons responsible to build capacity to manage their own NDIS plans and personal choices
- Empower participants to exercise choice and control over the supports received to assist the participant to achieve long term and short terms goals
- Educate participants, families, guardians, support staff and care givers about participant goals, care plans and service options
- Accurately document communications in accordance with NDIS guidelines
- Assess and obtain relevant NDIS required documentation with participants during intake and in the lead up to plan reviews
- Review participant support care plans, clinical assessments, professional and allied health reports to fulfill NDIS requirements in order to achieve the best possible outcome for participants
- Collaborate and contribute to NDIS pre-planning reports for team review with management prior to NDIA planning meetings
- Request updated and specific reports by professional and allied health personnel as required in support of NDIS submissions
- Support participants to:
- Assess a number of mainstream, community, informal and provider options
- Select preferred options or providers
- Negotiate services to be provided and their prices
- Develop service agreements and create service bookings with participant preferred providers
What we look for:
- A strong understanding of the National Disability Insurance Scheme (NDIS) and use of My Place system.
- An extensive understanding of service provision and related systems for people with psycho-social and other disabilities.
- Demonstrated extensive experience in Support Coordination.
- Superior level of communication skills and the ability to establish and maintain strategic working relationships with all key stakeholders.
- Demonstrated experience in building networks and referrals.
- Proven knowledge and experience in delivering a participant centered approach
- Understanding of the unique needs of people with disabilities, their caregivers, support services and their individual goals.
- Ability to navigate NDIS systems proactively ensuring participant needs are met
- Strong team work ethic and promoting positive experiences in the work environment.
- Excellent time management skills.
- A passion for helping vulnerable members of our community.
Other requirements:
- Appropriate graduate qualifications (Social Worker, Psychologist, Occupational Therapist, Other allied health, developmental educator, Social or Health Science Professional and experience in delivering support coordination for people with complex needs.
- Adhere to the NDIS Code of Conduct
- Adhere to the organisational Policies and Procedure
