Medication Incident Report

Do I need to report it to Bloomfield Care Services’ Office Care Team?

The term incident can be defined as an occurrence, condition, or situation arising in the course of work that resulted in or could have resulted in injuries, illnesses, damage to health, or fatalities.

Please ensure you have called or texted Clinical On Call Team on 0498 881 180 with notice of any medication incident prior to submitting this form.

As per our Incident Management Policy when a medication error is discovered the following steps must be taken:

  1. In the case of an Emergency DIAL “000” and follow instructions – offer First Aid until they arrive.
  2. Inform the BCS Clinical On Call team member (who will then inform the participant’s family or representative).
  3. Fill out this Medication Incident Report within 24hrs of incident or near miss occurring.
  4. Adhere to the outcomes provided by the BCS Clinical Manager.
